Philips Telehealth software release designed to help customers reduce readmissions and grow their programs
WHAT:
Philips introduces its latest telehealth software release at the annual meeting of the National Association for Home Care & Hospice. The new clinical application aims to change the way Philips’ customers monitor their patients at home and keep them out of the hospital.
WHY:
As the telehealth industry and healthcare landscape evolve, Philips Telehealth Solutions is innovating to continue to help improve access to quality care, reduce healthcare costs, and improve clinical outcomes. Part of the Philips Hospital to Home initiative, the new release of the telehealth clinical application includes features that will enable customers to more efficiently care for chronically ill patients, thus expanding the reach of their programs.
The software, which will be broadly available in December, also helps improve workflow by building on intervention management capabilities of the Philips solution. This will facilitate closer collaboration among care team members around an exacerbation—keeping patients under closer watch by multiple clinicians, until stable. The intervention management component helps enable swift and effective action, and ultimately improves clinical outcomes and reduces hospital readmissions.
The new release features:
- Expanded disease management capabilities, including diabetes, hypertension, and co-morbidities
- Enhanced clinical decision support with a higher level of patient workflow tracking and analysis
- Improved clinical data collection to support effective interventions
- New clinical outcome and patient population reports
