Philips Healthcare is voluntarily distributing an updated Quick Reference Guide (QRG) to owners of HeartStart HS1 (models M5066A, M5067A and M5068A) automated external defibrillators (AEDs) manufactured by Philips between December 2002 and November 2008. Philips has notified the appropriate Regulatory Agencies of this voluntary replacement of the Quick Reference Guide (QRG).
The QRG is intended as a supplement to the defibrillator voice instructions during use, if needed. It contains concise operating directions and its cover illustrates the three basic steps of using the AED. The QRG is typically stored in a clear plastic pocket inside the AED carry case for high visibility and ready access.
As part of Philips continuous improvement efforts, the cover of the updated QRG has been revised to emphasize the color and position of the orange Shock button. The cover of the previous QRG is black-and-white with slightly simpler graphics. In addition, the new QRG includes a page illustrating an alternative method to turn on the AED. The revisions to the QRG are intended to provide improved support, if needed, for the lay responder using the defibrillator in an emergency. This action applies only to the HeartStart HS1; no other Philips defibrillators are affected.
Philips will be replacing the QRG free of charge.
Identifying affected units
The HS1 defibrillators (models M5066A, M5067A, and M5068A) are sold in the United States as HeartStart OnSite and HeartStart Home, and internationally as HeartStart HS1, HeartStart First Aid, and HeartStart Home defibrillators. The revised QRG will be provided for all HS1 AEDs manufactured between December 2002 and November 2008. No other Philips defibrillators are affected by this action.
We are contacting directly all customers who, according to our records, own an HS1 that was shipped with a previous QRG. Customers will receive the updated QRG at no charge.
If you have not been contacted but are concerned that your device may be affected, we have provided a Serial Number Look-Up tool, at right. Simply enter the serial number of your HS1 defibrillator and click Check Serial Number(s). If your device is not on the list, no further action is required.
If your device is affected, you will be directed to contact Philips or complete and submit the Exchange form. Philips will arrange for the updated QRG to be shipped to you at no charge.
