It does this by providing a central repository for relevant cardiac information, including risk factors and family history. This cardiac information system provides information for educating and supporting patients through rehab clinics, heart failure clinics, etc.
The information recorded within the CVIS helps the patient, with the support of the healthcare team, take ownership of their road to recovery and beyond into the maintenance of their cardiac disease.
The aim is to prevent further acute events and possible re-admission to the hospital. Adherence to treatment and support in disease management has proven to reduce mortality.
